[Travelbee][Channels] To generate a QR code that when scanned by a requestor, it allows them to select which messaging channel to use to create/submit fault report tickets or service request forms

Currently, clients need to generate a separate QR code for each messaging channel to be accessed by requestors. Travelbee is requesting for a singular QR code that when accessed, shows the requestors all the integrated messaging channels available and allow them to select which is applicable for them.
