Currently you can only select either the standard Report Fault form or choose Service Request to display for this QR code.

QR codes are currently only able to link to "Fault report" or "Custom type" (ie. service request). To streamline our services, we've used the service request custom option and worked from there but it would be great if the QR could like to a page where there are options for "Fault report" and "Custom type"
