Configure the toilet feedback form to emulate the form from NTU
Notable differences:
- Specific question: "Are you satisfied with cleanliness of the washroom?"
- NTU has 5 ratings (Very good, Good, Satisfactory, Poor, Very poor
- Classification of defects (Amenities and Toilet)
- Comment section
- Allows image upload
